Coach’s Nutritional Screening Tool

The following screening tool is designed to help you identify potential problem situations with the athlete(s) you train. If you answer 'yes' to any of the following, your athlete(s) may benefit from a nutritional consultation with a dietitian who has met the criteria of the CAC Sport Nutrition Registry or who is affiliated with your Canadian Sport Centre.

Do any of the athletes you train . . . Yes No Not Sure
Live away from their families?
Prepare their own meals?
Avoid specific foods/food groups (e.g. avoid wheat, red meat, milk products)?
Follow a vegetarian diet?
Require a special diet (e.g. diabetes)?
Try to lose weight?
Compete in a weight class sport?
Try to gain weight?
Appear unusually fatigued before or during workouts?
Experience cramps (muscles or stomach) during training or competition sessions?
Get sick frequently (e.g. a cold or flu)?
Struggle with chronic injuries and/or illnesses?
Take a nutritional supplement?
Have a diagnosed or suspected nutrient deficiency?
Regularly travel out of town to train or compete?
Practice/compete in hot and/or humid environments?
Practice/compete in cold and/or dry environments.
Coach athletes 16 years of age or younger?
Coach a judged sport (e.g. figure skating, diving, synchronized swimming, ski jumping etc.)?
Coach a team sport?
Coach a sport which emphasizes a low body weight?
Discuss weight control with your athletes?
